LABORATORY/PHYSICAL EXAM REQUIREMENTS

(Incoming BSN/Midwifery 1st Year Including Transferees)

1. LATEST LABORATORY AND X-RAY;

• CBC
• Urinalysis and Fecalysis
• HEPATITIS B SCREENING or HBsAG
• CX-RAY-PA View

2. PHYSICAL EXAMINATION

Instructions:

• Undergo the laboratory and X-ray procedure at any hospital/laboratory of choice.
• Get the results and proceed to the Doctor’s clinic of choice for the physical exam.
• Secure the Doctor’s recommendation (Medical Certification) together with the laboratory and x-ray results and submit them to the school clinic for the next step.
